KENDALA RETENSI TERAPI ANTIRETROVIRAL (ARV) PADA PASIEN HIV/AIDS DI RSUD DR MOEWARDI (RSDM) SURAKARTA JAWA TENGAH

Abstract
Terapi Antiretroviral (ARV) bertujuan untuk mengurangi risiko penularan HIV/AIDS, menghambat perburukan infeksi oportunistik, meningkatkan kualitas hidup Orang DenganHIV/AIDS (ODHA) dengan menurunkan jumlah viral load dan meningkatkan jumlah CD4. Kunci keberhasilan terapi ARV adalah mempertahankan retensi. Lost To Follow Up (LTFU) dan kegagalan pengobatan menjadi kendala bagi ODHA dalam mencapai keberhasilan pengobatan ARV. Penelitian ini bertujuan untuk mengetahui prediktor hambatan mendapatkan retensi pengobatan ARV. Penelitian ini menggunakan rancangan case control dengan jumlah sampel 100 responden. Analisis data dilakukan dengan menggunakan metode chi-square. Hasil analisis bivariat, prediktor buruknya retensi berdasarkan persepsi pasien terhadap hambatan menjalani terapi ARV adalah kendala biaya transportasi (p=0.002; COR=8.34), kendala jarak perjalanan ke RSDM (p=0.002; COR=5.06), hambatan kesibukan lainnya (p=<0.001; COR=6,64), memiliki Pengawas Menelan Obat (p=0.003; COR=6.64), mengungkapkan status ODHA kepada orang lain (p=0.004; COR= 12.25), dan stigma (p=<0.001; COR= 12,3). Berdasarkan hasil uji multivariat persepsi pasien terhadap hambatan dalam menjalankan terapi ARV yang paling berdampak pada buruknya retensi terapi adalah mengungkapkan status ODHA kepada orang lain (AOR=13.1; p=0.042).
