ANALISIS RISIKO PADA DIVISI PERENCANAAN DAN PENGENDALIAN OPERASI MENGGUNAKAN METODE FAILURE MODE AND EFFECT ANALYSIS (FMEA) DAN FAULT TREE ANALYSIS (FTA) DI PT. XYZ

Abstract


PT XYZ is a manufacturing company engaged in the production of railway facilities. In the production process, one of the divisions that plays an important role in ensuring smooth operations is the Operations Planning and Control (PPO) division. This division is responsible for schedule planning, material procurement, and reporting. Based on information from division heads and staff in the Operations Planning and Control (PPO) division, there are often delays in project completion which have an impact on reducing operational performance and customer dissatisfaction. This study aims to identify and analyze the risks that occur in the PPO division and provide suggestions for improvement. The methods used are Failure Mode and Effect Analysis (FMEA) and Fault Tree Analysis (FTA). The results of the analysis using the FMEA method show that the risk with the highest Risk Priority Number (RPN) value is that the project is not completed on time or plan end, with an RPN value of 64. The risk was further analyzed using FTA to identify the root cause. Based on the fault tree diagram, two main basic events were found, namely delays in material delivery and problems in the production process. The results of this study were used to develop improvement recommendations aimed at improving the reliability of the production system and the effectiveness of operational planning at PT XYZ.
